After you complete the form received by the NFC, the New Patient Coordinator will contact you immediately to schedule new patient appointments.
Be aware: If you complete information on the web page you still need to give information your medical records from your previous doctor and a copy of the front and back of your insurance card.

Consultation fee is $ 110 to $ 425.
Laboratory tests, ultrasound, and other procedures will result in additional costs.
Your insurance will be verified before your appointment.
